Prior to obtaining a scheduled, live video chat and e-consultation with Dr. David Albin, potential patients will need to submit the Personal Video Consultation Request form below.
This form can be submitted vie email by clicking the 'submit' button at the bottom of the page. Once you have submitted your form, our office will be in contact with you to set up the video consultation.
The video consultation service requires a web cam and computer installed with SKYPE software, which is free to download here:
